In this webinar, the Q Exchange 6 team working on ‘Staying Close’ Transitions Nurse pilot project for care leavers will present their quality improvement project aimed at addressing the gap in health care services for care leavers. Young people transitioning out of care often experience health and wellbeing challenges without the familial support system that others rely on.
The team will explore how they are developing a health service designed to support care leavers, as they navigate this critical phase of life. The project focuses on co-designing services with care leavers to ensure their health needs are met and sustained, helping to prevent long-term health issues.
They’ll discuss how collaboration with the Leaving Care team, a focus on trust, and continued support beyond age 18 can improve engagement with health care and ultimately contribute to better health outcomes. They’ll also share their plans to measure success and the broader potential for this initiative to be replicated in other regions.
Speakers will include key project facilitators from the Children Looked After health team, and the QI team. They will cover:
- An overview of the project and its aims
- Co-designing health care support with care leavers
- Key learnings from the project so far
- The potential for national scalability
